How do I access the clinic?

Print and fill out referral paperwork, and send to HIV Alliance. Click here to download the self-referral form in English. (en español)
Another option is to be referred by your Case Manager. You will be asked about your dental history and you will be asked to fill out a few forms. Talk with your Case Manager today!
